Three Years After A Doctor Leaving a Public Hospital

Three years ago, Dr. L resigned from a famous Grade A hospital in Beijing where he worked for more than 20 years and joined a high-end private hospital. After working in a private hospital for more than three years, he chose to resign and return to the public Grade A hospital.

For the sake of convenience, we call the public hospital where Dr. L resigned as the B hospital, the private hospital he joined as the H hospital, and the public hospital where he returned is called the C hospital. I am a neurologist.

When I resigned, I was already in the title of senior high school. In 2012, Jin Zhenggao was generally considered early before the age of 40. I delayed for a year and was promoted to Jin at the age of 40. In the department, I was also highly valued by the director and always served as his deputy and as the deputy director of the administration. Everyone believes that I have passed the most difficult stage in the doctor's career and the future is bright.

The dean also felt that my resignation was very sudden and he kept it many times. She told me that even if you must leave, the H hospital is not suitable for you. Personal development and platform do have a very important relationship, so you must consider it carefully.

To be honest, I made a very determined decision to make this decision.

The profession of doctor is my ideal since childhood. I was in poor health since I was a child and often got sick. I remember that when I went to the park, I would have a fever when I came back, so I often went to and from the hospital. Since I was a child, I have been familiar with the environment of the hospital and felt that the doctor was amazing and could treat people.

So I wanted to be a doctor since I was a child. When I was a child, my toys were all stethoscopes, syringes, etc. When I filled out the college entrance examination application, I basically filled in medical schools. Finally, I was admitted to Capital Medical University. My doctorate was studied in the Medical School of Peking University. After graduating from my doctorate, I stayed at the Peking Medical Teaching Hospital B Hospital.

For others, a doctor may be just a career that makes a living, but for me, besides making a living, it is also the realization of my wishes. This profession has a special significance to me.

After I worked as a doctor for more than 20 years, I found that although I was very busy, I couldn’t take a few weekends a year, but the time I spent on clinical practice has become less and less.

Throughout the year, I almost always attend various academic conferences outside on weekends. Some of these conferences have gained something, but sometimes there are some repetitive conferences. You know, Chinese society is about human feelings, and it is impossible to refuse if you ask for support. In the academic circle, these are inevitable.

I feel that the time spent on medical treatment is getting less and less, and I think this has gone against my original intention of being a doctor. In addition, I am always having meetings outside on weekends, which makes me tired of dealing with it.

Another reason that prompted me to leave was that the doctor's practice environment was getting worse and worse. When I was hospitalized, I went to a consultation at night. When the patient's family besieged the doctor who asked me for consultation, I went up and pulled me. The patient's family beat me up and broke my nose and shed a lot of blood. But that was a relatively early thing, and no media reported the news of the injury to the doctor at that time.

Later in 2003, the doctor's status was significantly improved. I thought that the doctor's status could be maintained after the epidemic ended, but in fact, after the epidemic passed, the doctor's situation was worse than before, and various violent incidents of injuring doctors were frequently reported. Including the old director of our department, an old lady in her 70s, who was pushed down by the patient, had a compressed lumbar spine fracture and was bedridden for a long time.

These incidents of injuring and killing doctors made me feel quite disappointed. This is also one of the reasons that prompted me to resign from public hospitals, because private hospitals seem to have never exposed incidents of injuring doctors. Moreover, I heard that H Hospital has a system to protect the safety of doctors' practice. If patients have aggressive behavior towards doctors, doctors can refuse to provide medical services. I think this is quite reasonable.

Another reason for my resignation is the income problem. I am already high and I am also the deputy director of the department. If I go to some gray areas, my income may be much higher than in H hospitals. But I am a doctor to realize my childhood ideals, and I do not allow gray income to tarnish my ideals. Moreover, I also have faith, believe in reincarnation and cause and effect, so I choose to be a doctor in a regular manner, and this income is indeed not much.

Although the annual salary of doctors in public hospitals was increasing at that time, the wage growth rate was far from keeping up with the growth rate of housing prices. I have been a doctor for more than 20 years and have achieved this position. I still live in affordable housing in the suburbs outside the Fifth Ring Road. The commuting time is more than two hours a day, and my energy is exhausted a lot.

I remember it very clearly that at the end of 2017, I had two flus, and after a high fever, I felt that my physical strength had reached a limit. When I went to the H-hospital hospital, my income could not only increase by 50%, but also closer to home, and my commuting time on the road could be reduced to less than an hour. These are some specific objective factors. It seems not important, but it is actually impossible to ignore it completely. After all, life is not for others to see. After going to the H-hospital hospital, I felt really good at the beginning.

First of all, 80%-90% of my working hours are used in clinical practice, and I have sufficient communication with each patient. The hospital also stipulates that the first visit is 40 minutes and the follow-up visit is 20 minutes. No one comes in to disturb me, and the doctor is not allowed to answer private calls. It can be said that the hospital provides a good communication environment for doctors and patients.

There are too many patients in public hospitals and doctors are too busy. We often pay more attention to diseases and ignore patients. But in fact, the treatment target should not be just diseases, but the patient as a whole. Neurology now has a sub-specialty called neuropsychology. Many patients with headache and dizziness are not organic diseases, but may be psychological problems, such as anxiety and depression. As long as the doctor has sufficient time to communicate with the patient, he can find the specific cause and then provide some guidance from the psychological level, which will be of great help to the patient.

In addition, H hospitals are relatively standardized in private hospitals and emphasize evidence-based medicine. But later on, after a long time, some problems were found. One is that in various departments, in order to complete KPI, they compete with each other for patients. In fact, this may not be a problem in itself. To be honest, in public hospitals, some patients with many complications and older age are unwilling to accept them because doctors in public hospitals are afraid of medical disputes and are afraid of affecting turnover. These patients will have psychological burdens when receiving them. But in H hospitals, no matter how critical the patients are, everyone is scrambling, because the more critical the condition, the higher the overall medical expenses will be, and the easier it is to complete KPI.

But too much is too little. In order to complete the KPI, some departments go to grab patients who do not belong to the department, which will cause damage to the interests of patients. This is something I cannot accept.

After this departure and return, I should stabilize in the future and become a doctor safely unless the medical system of our country has undergone fundamentally tremendous changes. But from the current overall situation, this change should not happen for a long time, and the status of public hospitals will become increasingly stable in the future.

The doctor rushed to perform surgery on the patient on the wedding day
