456. Acute epiglottitis?

"Teacher Luo, the prescription has been copied, please read it."

A female college student who was intern in the emergency room handed over the prescription form and interrupted Luo Yinan's disease course record. She glanced quickly, nodded, and then knocked down a red worker seal: "Let me give it to my family and let them prescribe medicine as soon as possible."

"good."

Having written this, Luo Yinan sighed and leaned back on the back of the chair: "Is epiglottis recurring?"

"Little Luo, what are you thinking about?"

"The premature baby with biliary atresia in the west is here." Luo Yinan replied.

"Oh, I'm sent to the surgery department. How is the situation? Is it serious?" The person who asked was a middle-aged male doctor who walked into the emergency room with his Mediterranean head.

"It's quite serious to see the medical record materials." Luo Yinan pointed to the medical record book at hand and said, "But the person didn't send it."

"Hmm? What's wrong?"

Luo Yinan said the previous rescue process again and said: "After the surgeon evaluated, he decided not to be admitted to the ward for the time being, and he would wait until the infection was resolved in the emergency room."

"Infections can also be treated in surgical wards, why should we place an emergency room?" The male doctor didn't understand, "This is not an infectious ward."

"It is said that there is a suspected hospital infection in the west of the city. Maybe the child was also infected." Luo Yinan shook his head helplessly, "You also know what kind of children are in the surgery department. Every day after the operation, I feel like walking on thin ice, and I'm afraid of a complication or something."

"That's true. The chance of success of the surgery for biliary atresia was not high. If you put on the infection, you might not even get off the operating table at that time, and it would be of no use to collect it now." The male doctor also sighed, "It's just that her parents have suffered, and they have to wait at the door of the heavy cell to collect critical illness notices."

After explaining these, Luo Yinan's medical records continue:

"Teacher Luo, the prescription is given to the children's parents."

"Did the ENT department call? When will it come down?"

"I called, I said it would take ten minutes."

"It's ten minutes in ten minutes, and I called the surgical person, and evaluated the severity of biliary atresia."

"OK."

Luo Yinan looked at the intern who was about to walk out of the door, thought for a while, and called her back: "You ask the nurse to prepare the air cutting bag first, and choose the smallest one. If it recurs again, let the trachea be directly cut off by the ear, nose and throat."

The intern thought that the conservative treatment he copied by himself would work, but Luo Yinan even prepared the last method: "Teacher Luo, are you worried that she will relapse?"

"This is a recurrence, I had a dyspnea last night."

"Have you used antibiotics?" the middle-aged male doctor asked with his head raised.

"The penicillin used temporarily is the gentlest to the liver."

"It seems useless." The male doctor asked again, "How about the blood report?"

"I want to use cephalosporin for obvious infection, but..."

The liver and kidneys of newborns have not developed, and their abilities are limited. The toxicity of antibiotics will be amplified by this physical condition, so generally, the use of antibiotics is very cautious. The most common one is penicillin, which has little toxicity, but penicillin has limitations and sometimes it has to be used in combination with the second and third generation of cephalosporins.

Unfortunately, cephalosporin needs to be metabolized by the liver and is very effective for liver and gallbladder infection, but biliary atresia cuts off the path of excretion after metabolism, which is very troublesome.

"Do you want to come and have a look at Director Lu?"

"No hurry, I still want to try using cephalosporin to reduce the dose." Luo Yinan thought for a while and then took out the prescription order around him. "At worst, you can increase the interval between use and control the infection first."

"Anyway, you are in charge of the heavy cell now, so you can do it."

Luo Yinan finished writing the prescription in a simplified manner and continued to record the medical records:

Having written this, Luo Yinan stopped writing, looked at the last five words "acute epiglottis" and hesitated for a while, but then he typed "?" at the end.

"Teacher Luo, the prescription has been copied, please read it."
